We are happy to register new NHS patients. The registration procedure is on this page. We are also happy to update changes of address, name or contact details for patients who are already registered with us. To update your registered details, see below. Patients who would like to register at Gordon House Surgery If you would like to register at Gordon House Surgery as an NHS patient, you must be resident in our catchment area. Our catchment area is approximately within the following boundary roads: Scotch Common (to the North), Hanger Lane (to the East), Popes Lane (to the South) and Boston Road (to the West). Our catchment area includes some (but not all) addresses in postcodes: W13, W5 and W7. The residency rules are defined by the NHS. Once you are fully registered, your previous NHS GP medical records will be forwarded to our surgery in about six weeks. Please telephone our reception before downloading application forms, to check if you can be seen at our surgery: You need to be normally resident in our catchment area on at least 4 days per week for at least the next six months. If you do not meet the residency and catchment area rules, you may still be eligible for NHS treatment as a temporary patient or for emergency treatment.
Some patients in the UK temporarily from abroad are not entitled to NHS GP treatment for chronic conditions. This depends on the country of origin. Some UK nationals who are usually resident abroad may not meet the NHS residency rules for NHS GP registration, but can be seen in an emergency. For each family member, please complete the relevant forms (see below Adult & Child) Forms may be printed and hand written or you may want to complete the forms electronically. Filling in the Forms Electronically There are 2 options for each form, a .PDF file which you fill in electronically if you have 'Adobe Reader X' installed on your computer or a .RTF file which is compatable with Office in conjunction with 'Microsoft Word'. The .RTF file has tick boxes which can be checked electronically by right clicking on them, selecting properties and changing the default value to 'Checked' To download the form(s) right click and select 'save target as'. Save it to your computer, complete it electronically and then print and sign it.
The more information you give us, the easier and quicker it will be to receive your notes from your previous GP. If you have any difficulty in filling in the forms, please ask a member of our reception staff to help you.
